To request a referencing visit, please take into account the following steps:
Step 1:
Download and complete form
CC-SGI-FRM-026 for referencing
requests by other institutions.
Step 2:
Send the fully completed form to the email address director.calidad@clinicacolombiaes.com
Step 3:
Subsequent to sending the request in the defined format, Clínica Colombia will analyze and schedule the date and time according to the scope and topics requested.
Step 4:
The quality office will send a response to your request within a maximum period of 20 days, via email.
Step 5:
Prior to the visit, it is essential to send via email the list of visitors and the ARL (Workers' Compensation Insurance) affiliation in case the visit is in person.
Step 6:
Once the visit is concluded, we kindly ask that you complete our satisfaction survey.
