PATIENT SAFETY

Patient safety is the set of structural elements, processes, instruments and methodologies based on scientifically proven evidence that seek to minimize the risk of suffering an adverse event in the health care process or to minimize its consequences. Facilitating safer practices can be defined as the ability of an organization to identify, report and investigate incidents and take corrective actions that improve the patient care system and reduce the risk of recurrence.

Correct patient identification: The objective of this action is to improve the accuracy in the safe identification of patients in order to detect any risk that the patient may have during their hospital stay, likewise avoiding errors

in health care.

Improving effective communication

It is the communication that, through good communication skills and forms, achieves a quality and safety factor in health care.

Improve the safety of high-risk medications.

The safe administration of medications is one of the most frequent and impactful activities in our institution, for that reason we have standardized by means of checklists for the technical reception of medications, conservation and storage.

We have a procedure for marking high-risk medicines and similar, whose objective is to establish the necessary activities to guarantee the recognition of high-risk medicines and similar, in order to contribute to prevent problems related to their use.

Guarantee surgery in the right place, with the right procedure and patient.
Surgical procedures are of vital importance for our institution, therefore we have articulated the surgical units with the areas of hospitalization, emergency, diagnostic imaging and outpatient care through a surgical request form, we have an alert system that allows us to categorize patients for surgery according to their health status.

Reduce the risk of healthcare-associated infections.
1. We comply with the hand hygiene program and carry out two annual campaigns to strengthen this safe practice.
2. We prevent infections associated with the use of invasive devices by complying with established guidelines and protocols for the prevention of HEALTH CARE ASSOCIATED INFECTIONS (HAI).
3. We comply with biosecurity measures in the clinical management of our patients as established by the occupational health program of the Colombia Clinic.

Reduce the risk of harm to the patient caused by falls.
1. Identify the patient at risk of falling and re-evaluate the patient when indicated.
2. We place a BLUE sticker indicating the patient at risk of falling.
3. We apply preventive measures to avoid falls, educate the patient and his/her companion.

Prevent the appearance of pressure zones in the patient.
1. Identify risk factors for pressure ulcers.
2. We place a GREEN sticker that identifies the risk of pressure ulcers.
